ST BARNABAS: How do you get referred to St Barnabas?

Sometimes people aren't entirely sure about how patients are referred to the hospice, so I thought I would take the opportunity in my column this week to explain how our referral process works.

To be referred, you need to ask your GP, consultant or other healthcare professional to make a referral.

You may be referred to the service if you have an advanced or progressive terminal illness, from which you will not get better.

You may be referred for advice and support regarding symptom control, psychological support for you and your family, or for help in managing end of life care.

We aim to see urgent referrals within two days, and normally routine referrals are seen within ten working days.

A member of the team will contact you and offer you an appointment at the hospice or at home. This will usually last about an hour.

During this appointment we will find out how you are feeling and what impact your illness is having on you and those close to you.

We will also discuss measures to assist your current situation, and once an action plan has been agreed with you, we may make recommendations to your GP and/or district nursing team.

We may liaise with other healthcare professionals involved in your care, before we will agree with you, depending on your individual needs, how we will continue to support you.
